You have 2 choices.
i. Deploy MySQL as a worker role in your Azure compute instance.
ii. Migrate your database to a Microsoft SQL Server instance (SQL Server Express may be sufficient for your testing). Then deploy your data onto SQL Azure.

This talk at the Microsoft PDC 2009 conference sums it up best, and there is a video recording for your viewing too. microsoftpdc.com/Sessions/SVC51

If you are deploying your DB onto SQL Azure, please create just a 1GB instance.

I have completed deploy MySQL as worker role in Azure phpmyadmin-pma.cloudapp.net/phpmyadmin/index.php

How can I get the server name/IP address? I'm sure it's not localhost, right?

Yes you're right, of course it is not localhost. In the PHP video session, or if you have downloaded the slide, it says in Slide 20 that in order to consume MySQL you have to discover the IP address and port of the worker role instance on which you have deployed MySQL.

Easiest is to use the MySQL Solution Accelerator available for download @ code.msdn.microsoft.com/winazuremysqlphp....aspx?ReleaseId=3553

With that you can use command line to administer your MySQL, other features include
- Command-line access to worker role instances
- Server runs in each worker role instance
- Client runs in a web role
