أولا نبدأ جزء جزء لأن ده ذي اللي في الأمتحان
Normal values:
I- Haematology:
RBCs:
أول حاجة عايزين نعرف فيه
Anemia, Normal, Polycecemia
و لو أنيميا نوع الأنيميا اللي فيها يعني
Macrocytic hypochromic
Macrocytic normochromic
Normocytic normochromic
Microcytic microchromic
يبقي أولا لازم أول حاجة أشوفها هي ال
RBCs count
علشان نعرف فيه أنيما و لا طبيعية و لا زيادة يعني بولي ثيثيميا
تاني حاجة بقي لو هي أنيميا نشوف
-MCHC-----> To know either if it is hypochromic OR normochromic (There is no hyperchromic)
لو حصل و فيه
Hyperchromic---->
نتيجة حصل
hemolysis
و أنا بسحب العينة يعني
Error
-MCV----> To know the size of RBCs either if it is macrocytic, microcytic, normocytic
ده كده أول حاجة
طب لو فيه أنيميا عايز أعرف حاجة هل هي
Regenerative OR non-regenerative anemia
يبقي نشوف اييييه؟؟
1- Reticylocyte: If inc indicate reginerative
2- NRBCs: Nucleated RBCs----> Normally not present if present regenertive
و الأسباب بقي من النظري
طاااايب في حاجة بقي لو موجودة تدل علي حاجة معينة؟؟؟
لو لقيت في ال
Case----> sperocytosis: Means it is signs of autoimmune hemolytic anemia which result in destruction of RBCs
Anisocytosis----> طبيعي لأن هي أنيميا و رجنيراتيف فالأحجام مختلفة في خالية كبيرة و خلية صغيرة كده يعني ملهاش لازمة بس بتأكد علي الكلام

Calculate the red cell indices??

How to calculate??
Easy
MCV =pcv/rbcs x10 , MCH = hb/rbcs x10 , MCHC = bh/pcv x 100
(((((Imp.: HCt is the other name of PCV)))))

B) WBCs
أولا هو جزء بسيييط بس عاامل صداااع للناااس علي الفاضي هو بما أن معانا النورمال فاليو فحتي لو جاب عفريت أزرق يونت هيبقي النورمال عفريت أزرق برضووووو
طب قبل ما نبدأ في كذا حاجة سؤال مهم؟؟
1- Seg. neutrophiles-----> Normal, mature bec. Normally it is segmented
BUT
Hypersegmented this is the abnormal (See the lectures)
2- Absolute reticulocyte
Absolute means 2, 3, 4668, 34, رقم صحيح

و نقارن بالطبيعي نورمال فاليو

هتقول مين زياده و مين قليل و كده بسسسس
....penia (Dec. in number), ....cytosis (Inc. in number), Normal (Normal)
و في كده بقي دكتورة قالت ركزوا علي شوية حاجت اللي فاكرة منهم هو ال:
-Shift to left
-Write short notes on physiological neutrophilia
-Stress neutrophilia
يعني مش فاامر غبير كده

Case Example:
Platelets 88 * 10 to power 3/ul
First thing in this case the erythrogram is macrocytic hypochromic regenerative anemia.
erythrogram he mentioned that there is sperocytosis which is signs of autoimmuno hemolytic anemia which result in destruction of rbcs and cause the prehepatic jaundice.and also there is thrombocytopeia which also may be due to autoimmune thrombocytopenia.
About the leukogram i t is inflamatory neutropilia with regenrative shift left

Indication of problem in kidney by inc. the following parameters:
-Creatinine
-BUN
-Phosphorus
-Amylase
-Lipase
بالترتييييب كده يعني أهمممم واااحد هو ال

ممكن لوحدهم يدلوا ان في مشكلة في الكلية خصوصا ال
creatinine
لو كله زاد و هو مفيش زيادة مفيش مشكلة في الكلية؟؟؟
Urinanalysis:
لو دخل ال2 مع بعض بقي
High specific gravity + inc. Creatinie + Pus cell inc., RBCs, Albuminurea-----> Acute renal
Low specific gravity + inc. creatinine, few cast or 1, protein mild or +, albumin mild or +----> Chronic renal
