I need to create a Forum on my Facebok page with a separate tab for it. can I somehow add my forum to the facebook page?

Yes you can

To do that,

First create your Facebook page.
Click "Add to my Page", on this page http://www.facebook.com/forum5, on the top left.
Select your page.
Go to your page and you will see a "Forum" tab.
Click Forum and you will be provided an ID.
Go to http://www.facebookforums.net/create-a-forum-on-facebook/ and update your forum info providing your Facebook page.

Thats it.

Thank you! Another question. Can I change the tab title? I'd like to put my forum's title instead of the common "Forum"

No you cant because Facebook prevents that. You can choose "Forum" only.

Groups have no tab settings, so you cannot.

what about on my facebook profile?  right now I only have "[Wall] [Info] [Photos] [Friends]" but I've noticed that some people also have [Notes] and I think I've also seen [Boxes] and [Videos] for some friends.  I would love to have [Forum] directly on my profile.

I made my forum here http://www.facebook.com/digison?v=app_174481013789&ref=sgm
but after clicking on any thread i am no more on my profile page in FB. Can I anyhow make it still be there?
So that anytime and place using forum my profile photo and tabs are visible.

Actually no, but we will provide a new service for you guys.
Your own space at the top. You can put a banner and make money, or you can put your facebook page menu there.
So, your forum users can go back to your page easily.

I made a forum, and submitted the page ID. I went to http://www.facebook.com/forum5 but i found no "AD TO MY PAGE" link under the logo of the forum. What am i doing wrong?
